Student Information
DASH by Relay - Student Information
From “My Caseload”, click on the student’s name. Alternatively, you may click on the caret next to Log Service to open the pop up menu.
Student Demographics – Basic information imported from your district is displayed here. Information is only as accurate as the data received from your district.
Service Plans – Displays imported plan data for provider reference. Will include service frequency and duration. This information will also be displayed on the student’s service log at time of entry.
Goals and Objectives – Will display imported data or allow users to manually enter goals and objectives.
Note: Not all Dash by Relay systems will display Goals and Objectives. This is a state by state, client by client basis.
Logs – Displays services for individual students.
- Service Logs – Displays entries by you for that student. Logs that have not been submitted for Medicaid reimbursement will allow edits. The edit button is located to the far right.
Shared logs - Shared logs are visible to like provider types (OT to OT, SLP to SLP, etc.) when both providers have the same student on both of their Student Caseloads in Dash by Relay.
- Shared Logs - To view shared logs you will need to open the student information page. Scroll to the bottom of the page where your Service logs are available. Notice the two tabs. Click on the "Shared Logs" tab on the right.
